Supported Community Living

Passages' supported living programs provide individualized support to persons with disabilities to live and interact within their community. Individuals receive this instruction and care in small settings. Quality training to meet each person' s needs by experienced, dedicated staff members is geared to further development of independence in:

Admission Requirements

To be considered for funded living in the community,, an individual must be at least eighteen years old, and meet the following criteria:

  • have been diagnosed with a physical, mental and/or emotional disability.
  • meet eligibility requirements for funding sources such as Medicaid. Private pay is an option.
  • be capable of participating in services on a regular basis as recommended by the Individual Support team.

    Benefits
  • We have a dedicated, experienced staff who meet all federal requirements, are certified in medication administration and trained yearly in Crisis Prevention and Intervention, CPR, first aid and behavioral support.
  • Individualized training based on needs and choices with family and individual's input.
  • A Support Team of professionals for each individual to oversee and ensure quality programming.
  • Opportunities for a variety of leisure and recreational activities at home and in the community.
    Visits with family members are encouraged as desired.
  • On-going health care with agency RN, community doctors, psychiatrists and specialists.
  • Physical, Occupational and Speech Therapies as needed.
  • Encouraged visits with family as desired.

    Residential Care Choices:
  • Group Home - This setting provides 24 hour supervision for up to 8 individuals in homes that are fully handicapped accessible.
  • Supported Living - This setting can provide 24 hour supervision in smaller group settings depending on the needs of the individuals.
  • Independent Living - This setting provides minimal supervision to an individual who lives on their own in the community.
